Refund of Double Deductions |

| |
1. |
AUPE Multi-purpose Co-operative Society (hereinafter referred to as the ‘AUPE Co-operative’) has kindly agreed to assist its Muslim members by refunding any double deductions made by the AGD in November 2008. |
|

| |
2. |
Such a member may either: |
|
| |
|
|
| |
|
(a) |
Refund by Cash |
|
| |
|
|
| |
|
personally go to the office of the AUPE Co-operative at “Wisma AUPE“, 295 Upper Paya Lebar Road, Level 3, from Monday to Friday between 9 am to 7.00 pm; or |
|
| |
|
|
| |
|
(b) |
Refund by Cheque |
|
| |
|
|
| |
|
email or fax to the AUPE Co-operative for refund by cheque if he is unable to go to the office. |

3. |
The procedures, information and documents required are as follows: |

(a) |
Refund by Cash |
|
| |
|
|
| |
|
A member has to bring along his identity card and Nov 2008 payslip to show that there was a double deduction made; and |
|
| |
|
|
| |
|
(b) |
Refund by Cheque |
|
| |
|
|
| |
|
A member has to state in the email or fax his personal particulars like name, NRIC number, home address and mobile telephone number as well as his bank account number in which the refund should be deposited into. In addition, he also has to attach the Nov 2008 payslip and to state in the email or fax if he prefers: |

| |
|
(i) |
to come to the office of the AUPE Co-operative to collect the cheque; or |
|
| |
|
(ii) |
to have the cheque posted to his home; or |
|
| |
|
(iii) |
he would like to request the AUPE Co-operative to do a quick cheque deposit on his behalf. |

| |
|
|
| |
4. |
The AUPE Co-operative is extending its operations hours by 1 hour from 9 am to 6 pm to 9 am to 7 pm from 13 to 21 Nov 2008 as a special service to members so as to help them in the refund of the double deduction. |

| |
5. |
The closing date for the above-mentioned is 21 Nov 2008. If by the end of this date, there is no request for cash or cheque refund, the AUPE Co-operative will arrange to deposit the amount of double deductions into the affected members’ Savings Accounts with the AUPE Co-operative. |
